in accordance with the policy of the Catholic Church in Australia relating to professional standards state that there have not been any substantiated complaints of abuse against me and I am not aware of any circumstances that might give rise to a complaint of abuse against me regarding professional standards. I am aware that, in moving into a new jurisdiction, I will be subject to the requirements of relevant State legislation concerning clearance to work with certain categories of persons.

45.6 Whenever a cleric or religious is to transfer from one diocese or institute to another (whether within Australia or coming from overseas) or to carry out a ministry or apostalate in another dioscese or institute, the Church Authority to which the person is to be transferred shall ask for a written statement from the priest or religious indicating whether there have been any substantiated complaints of abuse against him or her or whether there are known circumstances that could lead to a complaint of abuse. Such statements shall be held as confidential documents by the Church Authority.

WORKING WITH CHILDREN CHECK (WWCC)

Those visiting Clergy who hold a NSW Working With Children Check number, please provide the WCC number below. For those Clergy who DO NOT hold a NSW Working With Children Check, please complete all the required information in this section.
A WWCC is a requirement for all clergy who minister for more than 30 days in a calendar year in NSW, Australia.

Note: the period of 30 days refers to the actual days spent providing religious services. e.g. Celebrating Mass, hearing confession, etc. If you are in NSW for more than 30 days but only provide religious services for 5 days, you will not require a NSW WWCC.

Please refer to the Office of the Children's Guardian webpage for further details on applying for a NSW WWCC.
